What to Bring to Your First Visit

• Driver’s license or another valid photo ID
• All insurance cards for current coverage
• A list of all prescription medications and vitamins you are currently taking
• Complete “New Patient” Form (link is on website) and bring a copy to your appointment
• A copy of your medical records from your previous doctor or primary care physician; if you don’t have these, you can ask your previous doctor to send them
• Prepare and bring a copy of your health concerns
